I've done an offline scan a couple times. Just now I logged in, Malwarebytes scanned and found nothing whereas Defender found the same threat in the same folder a few minutes afterwards, however the filename is different.

That looks like a DOC attachment to an Email in the Windows 10 Mail App.
MS Office Word DOC attachments can potentially contain a Macro virus.
Only you will know whether you send or receive mails with Word Docs with macros.
I assume that you use the Mail App. If you don't then uninstall the Mail App.
You can reinstall via the MS Store.

suggests the file is being downloaded by one of the Windows Communications or Store Apps.
When you check for mail, at least make sure there are no remnants of old emails left in your Junk/Spam/Deleted Folders.
If you are using the Windows Mail or Calendar App try disabling "Automatically Download External Content" to see if it makes any difference:

I'm going to try disabling external content just for the sake of testing, but it means a lot of my emails are indecipherable because they consist of images.
These days I do get a decent amount of phishing emails, I never used to, I'd guess it's someone else in the household signing up to funky shit and it's spread across the network. However, if Mail is to blame for automatically downloading email attachments then that's asinine.
