OK, I found the option to turn off real time protection, MS seems to have changed the design of the page - you have to go to "Windows Defender Security Centre", then click the shield icon on the left, then click "Virus & Threat Protection settings", then there is a button to switch real time protection off, but if I turn it off, as soon as I restart, it's turned back on again

Turn On or Off Windows Defender Real-time Protection in Windows 10 Windows 10 Tutorials

Scroll down to OPTION SIX in this Tutorial and download both the REG files. I use them when I want to disable OR enable Windows Defender. It only take a few seconds after I use either one for it to turn off or turn on.

I notice that MsMpEng.exe is still running, is that normal?

I see it running on my PC also. I wouldn't worry about it. Maybe another with more information might chime in here on that.

I can tell it is not actually running whether that shows or not. It causes lag in a few things on my PC but I do not see that lag once I ran the REG files. Hope that helps.

It is Windows Defender's service. It is not recommended to disable it, unless it causes problems.
