How long should I wait (its now been almost 3 hours at 95%) before trying something else like a manual download?

It took me about 15 minutes. I noticed when the computer was restarting that my log in picture is back.

So far, so good. Windows Update Cleanup in Cleanmgr.exe (Disk Cleanup) is 1.34GB, so that definitely confirms the "growing size" of cumulative updates. Seems to be working OK on my Lenovo notebooks and Surface Pro 3. I'm waiting until after the workday ends to update my production desktop. At least, Patch Tuesday/Update Tuesday remains fairly predictable!

1.34 GBs is the same size as my update. A little large for anyone on a limited data plan.

Yes these large patches will be the norm. Each update is a cumulative update and replaces the previous ones. Was this way in 1507, 1511 and now 1607. They will only get larger and larger.
Windows is supposed to check which parts of a cumulative update you already have installed, and only update the new stuff. This is the description of the fix - https://support.microsoft.com/en-us/kb/3189866 - in which it says

"If you have installed previous updates, only the new fixes that are contained in this package will be downloaded and installed to your computer. If you are installing a Windows 10 update package for the first time, the package for the x86 version is 255 MB and the package for the x64 version is 431 MB."

So I'm not sure why there's such a huge download.

I think most of this package is new, hence the large download.

I'm not sure but could it be that 'package' is used to get the files necessary to install the update?

    KB3189866 stuck at 95%

Been stuck at 95% for over an hr. All updates went fine until KB3189866; Procdump grabbed crash dumps from ShellExperienceHost.exe and svchost.exe (hosting wuau) during updates.
