I had a similar problem on Windows 8. Following your directions, I observed the following config.

    (1) Device Association Service set to Automatic
    (2) Device Install Service set to Manual
    (4) Device Setup Manager set to Disabled

From your solution the following are missing

    (3) Device Management Enrollment Service
    (5) DevQuery Background Discovery Broker

The allowable values for the three items present (1,2,4) are
auto delayed start, auto, manual, disabled.

I set (4) to manual and started it but no joy

I then rebooted and my printer is now shown in the control panel! And further more, I can set it as the default printer.

    THANKS VERY MUCH!

    How I Fixed It....

This is how I fixed it. The printer shows in device manager but not in Devices and Printers. If you open Wordpad, try to print and see your printer, Right-click on it and click Properties. Go to the ports tab. If the printer is checked as DOT4_001 or not on USB001 (or 002), Check the USB001 box and click apply. This popped my printer back to Devices and Printers. Hope this helps somebody.
